you can blame Michael Dorn and John Logan for Worf's appearance in Nemesis. Logan wrote it and put Worf back on the ship, acting like he'd never left. Dorn was asked which he wanted to wear, ambassador's costume or his 'space suit' Starfleet uni and he said the uniform.

the novels have explained that he resigned as ambassador after the new UFP president was elected - as legally required - but didn't take up the post again, instead deciding to rejoin Starfleet and is now Picard's #1 on the Enterprise-E.

This is one reason why I've given up on Sci-Fi shows, I'm too old to worry about all this confusion to story lines & charater history, caused by parallel universes or stuff thats not in the film or T.V. series, but in some spin-off book that I'm never likely to ever read..I took a look @ that Wookiepidia thing, Damn!, some people take this nonsense way to seriously!

Besides, how easy is it to say "It's a parallel universe", when in fact the author's just made a mistake, or can't be arsed to do some research

Couldn't agree more (though I might've worded it slightly differently, ). Some of my favourite Trek novels include Enterprise - The First Adventure, Strangers from the Sky, and Final Frontier, all of whose cointinuity has been contradicted by the films. Doesn't stop me enjoying these cracking good stories.
